Medical Director
Our hospice medical director will work with your primary physician to ensure that your care is focused on your individual goals. Our hospice physician will coordinate all of your care needs if you do not have a primary care physician.
Hospice Nurse
Our HOPE Healthcare and Hospice nurses work side by side with your primary care physician and our medical director to provide your care needs. Your nurse will manage your symptoms by ensuring that your treatments are effective. They are available to you 24 hours a day.
Social Worker
Our social workers help you and your family members navigate all concerns that arise during this journey. The HOPE Healthcare and Hospice social worker can assist you with community, financial, and supportive resources.
Chaplains are available to provide emotional and spiritual counseling to you and your loved ones, regardless of your faith. They can also coordinate care with the clergy of your choice.
Nurse Practitioner
Our nurse practitioners will work closely with our hospice team to manage any of your pain or symptom needs.
Hospice Aide
Hospice aides are available to help you with activities of daily living such as bathing, dressing, or grooming. Your needs and plan of care determine the length and frequency of your visits.
Our registered dietician is available to you and your family to review any nutritional needs.
Physical, Occupational, and Speech Therapist
Therapists are available to consult with you and your family on any of your physical,
occupational, or speech therapy needs.
Our volunteers are available to support you and your caregiver with various needs such as sitting with you to provide companionship or preparing light meals.
Bereavement Coordinator
HOPE Healthcare and Hospice bereavement coordinators provide support for your family and loved ones. They will coordinate your physical, emotional, therapeutic, and spiritual needs.
